An unaccompanied Minor / Special Needs form for a special needs passenger or minor younger than 16-years-old is required to be on file at the Ferndale Airporter Office in order for a minor to travel on Bellair Airporter Shuttle.
To assist in the safety of minor passenger(s) traveling alone, it is recommended that they be seated in the front of the vehicle and avoid conversation with anyone other than the driver. Bellair Charters / Airporter Shuttle does not assume responsibility for the passenger once the passenger exits the vehicle, whether this is at an intermediary stop or the final destination. Also, Bellair Charters / Airporter Shuttle is specifically not assuming the role of a guardian of the minor or special needs passenger.
Note: Children between the ages of 12 to 15-years-old may travel on Bellair Airporter Shuttle without a parent or guardian. Children less than 12-years-old are required to be accompanied by a parent or guardian.
Please fill out the below form to authorize the transportation of your Minor / Special Needs passenger. You must be the parent or legal guardian of the passenger to complete this form.
